New changes in Medicare continue for documentation review

-RAC audits are being utilized by the Federal Government to scrutinize physical therapy records as a way to search for "errors and ommissions" as a way to get further aid in funding Medicare cuts.

On average a RAC auditor is compensated 9.5-12.5% of the dollar amount that has to be returned to Medicare by the home health agency.

Example: $1,000 returned to Medicare ie: missing date on a visit note, and $120 will be paid to the RAC auditor.

Bottom Line: Proper and defensible documentation is key for physical therapists to prevent costing themselves and the agencies they work for hundreds, and sometimes, thousands of dollars in returned revenue.
